🎥 Live Streaming Consent Forms

At Sacred Heart Catholic Church, we occasionally live stream services via YouTube to allow those who are housebound, distant, or otherwise unable to attend in person to take part in the liturgy. To protect the privacy of individuals who appear in or support these services, we ask for consent from those in key visible roles such as:

• Altar servers
• Readers
• Eucharistic ministers
• Musicians and singers
• Anyone taking part in a celebration being live streamed

Consent is especially important for services such as funerals, weddings, christenings, and celebration Masses, which may be privately streamed via unlisted YouTube links.

🧾 Who Needs to Complete a Consent Form?

• Adults who will appear on camera or serve in the liturgy (e.g. ministers, readers)
• Parents or guardians of anyone under 18 who will be visible on the live stream (e.g. altar servers)

If you are unsure whether you need to complete a form, please contact the parish office for guidance.
